If you are not yet registered, please read the instructions below and download the appropriate registration forms to become a member of USA Boxing. There is now an online registration option for members and potential members in these LBCS:

Renew - (Renewing Members Only, MUST HAVE AN online account before renewing)
The instructions below are for non-online registration:
To become a non-athlete, athlete, master athlete or club member please complete the appropriate forms shown below and send to your Local Boxing Committee for processing. If you don't know who your Local Boxing Committee is, please refer to Local Boxing Committee (LBC) Directory by clicking HERE.
Non-athletes: If you would like to register as a non-athlete please complete the forms listed below, sign them and send the forms to your local boxing committee with the appropriate registration fee. Back ground screening is required every other year. If you completed a background screening for 2010 than you do not need another one till 2012.
Athletes: If you would like to register as an athlete of USA Boxing you must be at least 8 years old and follow the instructions below to provide the their local boxing committee with the appropriate registration fee, documentation and membership application. If they are under 18 years of age, the application must be signed by a parent or legal guardian.
- New athletes registering must provide two passport size photos and original birth certificate for proof of age with their membership application and appropriate registration fee before their application can be considered complete.
- Renewing athletes must provide their old passbook, membership application and appropriate registration fee before their application can be considered complete.
Master Athletes: If you would like to register as a Master Athletes you must provide the same information as all athlete members (new or renewing). They must also provide the results for their physical examination by their personal physician. They must receive a physical exam each year they are registered.
Club: If you would like to register as a club please fill out the club membership application shown below and send to your local boxing committee along with the appropriate registration fee for processing. Accident Insurance Forms: If necessary insurance forms and boxing injury report must be filled out completely and verified by a local boxing committee representative. For further details on this process, please see the filing instructions document shown below.

Liability Certificate: Certificate of Liability must be filled out completely and submitted to your local boxing committee in the appropriate circumstances. For further details, please see the Certificate of Liability instructions document shown below.
