To all Level II and Level III Officials, Re: 2010 USAB Sr National Championships. The officials task force would like to remind each of you that USAB has set a 12 pm/noon deadline, MST/Colorado time on Monday June 14, 2010 for the receipt of all necessary forms/paperwork for Level II and/or Level III officials wishing to officiate at this upcoming event. You can obtain the necessary forms at www.usaboxing.org.

Click 2010 Senior National Championships and go to Attachments on the right hand side. Under attachments, click each of the forms necessary for you to complete/fill out:

• Non-athlete entry form;
• USOTC Participant Biography;
• USAB Code of Conduct;
• USOTC Waiver and Release of Liability;
• HIPPA signature form;
• Officials Code of Ethics;

• Officials Referral Form (must be signed by both your: LBC President and LBC COO)

IMPORTANT:   All of the above completed and signed paperwork must arrive at USAB not later than 12pm/noon, MST/Colorado time on Monday June 14, 2010.
